Hospital Discharge Care

Hospital discharge care is a coordinated, person-centred approach that supports NDIS participants during the critical transition from hospital to home, transitional accommodation, or an alternative living arrangement. It ensures a smooth and safe discharge through early planning, tailored discharge plans, and close collaboration with hospital teams, allied health professionals, and NDIS coordinators.

Supports may include organising equipment, preparing the home or temporary accommodation, medication management, transport, and short-term in-home or supported accommodation assistance with personal care and daily tasks. CarePath Support also offers immediate, short-term and transitional accommodation—ideal for hospital discharge, crisis placements, and bridging into long-term SIL—with urgent referrals accepted and fast response within 24 hours.

By putting the right supports in place before discharge, this approach reduces the risk of complications and readmission, eases pressure on families and carers, and promotes safer recovery in a familiar or supported setting with clear, compliant NDIS documentation and timely approvals.

Independence in shared homes

Supported Independent Living

Our Supported Independent Living (SIL) service offers structured, personalised support in comfortable shared homes in Alice Springs and Melbourne. Participants receive assistance with daily routines, personal care, meal preparation, household tasks, and emotional wellbeing, all designed to promote confidence and safety.

We work with participants to develop life skills, maintain routines, and engage with the community. Our goal is to create a supportive home environment where independence can grow, while providing guidance and encouragement tailored to each person’s needs.

Guidance for your NDIS plan

Support Coordination

We help participants and their families understand and navigate their NDIS plan. Our team assists with connecting to providers, organising services, managing budgets, coordinating appointments, and reviewing plans to ensure participants receive the support they need.

We provide clear communication and personalised guidance at every step, helping participants feel confident and informed. By coordinating multiple services and liaising with Support Coordinators and plan managers, we ensure a seamless, stress-free experience.

Travel and Transport Assistance

Our transport assistance supports participants to attend appointments, community programs, social events, work commitments, and daily errands. We provide safe, comfortable vehicles and staff assistance, ensuring participants can travel confidently and independently.

Short Term Respite

Development of Life Skills

We provide coaching to improve essential skills such as cooking, budgeting, communication, decision making, personal care, and organisation. Our staff work closely with participants to practise and reinforce these skills in a supportive environment.

Access and Maintain Employment
